  1. Firm’s registered name and main business address
  2. Firm’s registration number
  3. Network Details*
  4. Regulatory body and authorisation number
  5. Holding company details*
  6. Name of service provider*
  7. Trading name*
  8. Branch address details*
  9. Details of all Directors, Partners, Principals* (Inc. name, position held, NI No. and DOB)**
  10. Email Addresses of all people within your organisation who may write business with us
  11. Bank Details (Sort Code, Account Number, Account Name, Name of Bank, IBAN No., Swift)
